Yeah, Jesus Camp takes the cake. That one made me want to go hide in a cave and never come out.
I think the scariest horror flick I've ever seen though is still The Changeling, one of George C. Scott's last flicks. Really creepy music and even creepier camera work. That movie has made me yell out loud more than any other. [SPOILER ALERT] I just got goosebumps thinking about George C. Scott finding that kid's room in the attic and opening the cobweb covered music box to hear it play the song he had just finished composing on the piano.
There's plenty of movies that have scenes that scare the shit out of me, like that scene in The Eye when that inky looking girl flies across the classroom, but overall The Changeling is still #1.
Oh, and then there's Audition. Not as scary, but oooooh so deliciously fucked up!

Honestly, in no particular order, that would be :
Them (the most stressful movie I've ever seen)
The Descent (this cave creatures mixed with the claustrophobic atmosphere, that was frightening)
Ringu (of course, was the first Japan Horror movie I've seen)

I liked Descent, mebbe too conventional at the beginning, but soon it has been bringing forth new interesting changes of action. The creatures were very professionally and nicely made, at least were not reminding of any other well-known personae, so quite inventive.

I got the creeps during The Grudge when the police officer was watching the surveilance tapes and that creepy girl stared back at him through the camera.

Eyes of Fire - everything Blair Witch wanted to be and then some.
Silent Hill - I didn't even realize it was a horror movie when I rented it. Irritated me at the beginning and I had to resist the urge to hit the eject button up until the female cop stops the woman on the road to Silent Hill and that thing comes staggering down the road - pretty disturbing after that.

Watched Guinea Pig series: 2, and 4. This is CRAP! Has nothing to do with traditions of J-Horror, has nothing to do with being scary or funny or whatthefuckever, ultimate bullxit! If you haven't seen it, please, do not bother.
And the so-called "special" graphic effects are so weak, that only an idiot would take these "movies" for real as snuff ones.

"Reincarnation" (Rinne) is superb, much better than previous ones, or "Ju-On". The doll is really creepy and the ending is so unexpected, that one might be shocked.
Because the hotel plays a significant role in it, I would see it a sort of new dimension's of Kubrick's "The Shining".

The Truman Show is still the only movie that scared the fuck out of me while watching it, and I wanted to go hide under a blanket. Another wonderful movie (since I noticed at least one Miike film mentioned) that may make you feel a bit funny is Visitor Q. This is my first suggestion to everyone looking for a good Japanese film, and by one of the best filmmakers out there.
Reincarnation was ok, but I am getting sick of all of these films about films. This one goes one step further: it is a film about a film from the perspective of someone filming their actions. My wife suggested it to me, but she isn't into the fucked up xit I prefer.
